We offer both Skilled Nursing Care and Long-term Care.
Skilled Nursing Care
If you are in a hospital and need therapy, you may qualify for Skilled Nursing Care. If you have Medicare, it may help cover the cost of Skilled Nursing Care for up to 100 days. A Skilled Nursing Facility (SNF) will help you recuperate and rehabilitate until you are strong enough to go home. A 3-day hospital stay is required in order to qualify for these services.
Services covered by Medicare, but not limited to:
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- IV Therapy
- Enteral Feedings
- Medications
- Social Services
- Medical Equipment
Placement in a SNF will begin with an assessment by a licensed nurse who will determine if we are able to meet your needs. Please ask your social worker or case manager to contact us at 915-566-2111.
Long-term Care
If you can no longer take care of your needs in a home setting and you are declining in at least two of six Activities of Daily Living (ADLS)* Then Long-term Care (LTC) may be an option for you. LTC includes:
- Assistance with ADLs
- Meal Service
- Medication Management
- Planned Activities
- Daily housekeeping
- Laundry Service
*Activities of Daily Living (ADLs) include activities such as bathing, dressing, eating, transferring, toileting, and continence care.
