We offer both Skilled Nursing Care and Long-term Care.<\/p>

\u00a0<\/p>

Skilled Nursing Care<\/strong><\/p>

If you are in a hospital and need therapy, you may qualify for Skilled Nursing Care. If you have Medicare, it may help cover the cost of Skilled Nursing Care for up to 100 days. A Skilled Nursing Facility (SNF) will help you recuperate and rehabilitate until you are strong enough to go home. A 3-day hospital stay is required in order to qualify for these services.<\/p>

Services covered by Medicare, but not limited to:<\/p>

  • Physical Therapy<\/li>
  • Occupational Therapy<\/li>
  • Speech Therapy<\/li>
  • IV Therapy<\/li>
  • Enteral Feedings<\/li>
  • Medications<\/li>
  • Social Services<\/li>
  • Medical Equipment<\/li><\/ul>

    Placement in a SNF will begin with an assessment by a licensed nurse who will determine if we are able to meet your needs. Please ask your social worker or case manager to contact us at 915-566-2111.<\/p>

    Long-term Care<\/strong><\/p>

    If you can no longer take care of your needs in a home setting and you are declining in at least two of six Activities of Daily Living (ADLS)* Then Long-term Care (LTC) may be an option for you. LTC includes:<\/p>

    • Assistance with ADLs<\/li>
    • Meal Service<\/li>
    • Medication Management<\/li>
    • Planned Activities<\/li>
    • Daily housekeeping<\/li>
    • Laundry Service<\/li><\/ul>

      *Activities of Daily Living (ADLs) include activities such as bathing, dressing, eating, transferring, toileting, and continence care.<\/p>
